It was obvious to people knowledgeable about gymnastics that she was lost in the air.

How was it obvious? What is the difference between the look of an athlete getting lost in the air and the look of an athlete suffering from performance anxiety because all eyes are on them?

She was suffering from performance anxiety (Twisties), which unlike in the stick and ball sports, can be deadly.

Many women have died/been paralyzed in gymnastics incidents.

Name five over the last 50 years at the Olympics.

Not all at the Olympics, but all because they messed up.

Adriana Duffy, Puerto Rico, 1989 Worlds, vault, paralyzed.

Sang LAN, China, 1998 Goodwill Games, vault, paralyzed.

Elena Mukhina, USSR, 1980 Olympics, floor, quadriplegic, died from complications.

Julissa Gomez, USA, 1988 World Sports Fair in Japan, vault, quadriplegic, died from completions.

Melanie Coleman, USA, NCAA gymnast at UConn, 2019, bars, died.

Taylor Lindsay-Noel, Canada, elite Olympic hopeful for London, 2008, bars, training in gym. Quadreplegic.

Mukhina and Gomez are the most well known of these. Coleman is most recent. There are many who have been paralyzed, mostly on vault, the event that truly made Simone withdraw.

Is this enough of them? It was extremely easy to put this list together as 4 of them I’d already heard of.

Every gymnast that watched her live and commented live, or just after the event and before any pressers, said she looked lost. Tim Daggett, who is a gold medalist, said "oh no" as she came down on the vault landing and then said, "she had no idea where she was". Laurie Hernandez was in studio and was asked and she said the same thing. The people that know what they're looking at spotted it immediately.
