My wife is a physician. I call her Dr. Wifey sometimes.

She hates it.

Outside of a doctor's office/hospital, a professional setting, or a classroom, you probably shouldn't expect people to call you Doctor. If a dude insisted I refer to him as Doctor at all times, I would probably avoid his companionship.

If you earned the degree, go for it. I have no problem with it. It does depend on the setting, however. Grow up in a small university town and most everyone's dad is a Doctor SoAndSo.

My dad had a doctorate. I can't remember him ever introducing himself as Doctor SoAndSo, but most people, except friends, called him Dr.

Now I'm imagining OP's guy correcting some waitress as he picks up his to-go order from a restaurant.

"I've got an order for Mike Smith"

"Ahem, it's DOCTOR Smith, actually."

I'd dump his food in the trash.

I don't call doctors Dr. when I interact with them at all, haven't in decades. I'm not concerned with whether or not a person I'm paying for a service likes how I address them.

I think if the person were in academia or research and it’s in an academic setting, sure. Somehow that seems like normal usage. If you’re a microbiologist, PhD and we’re at a bar, it seems a bit out of place. I think the relationship matters too.

The doctors who I know like that, I wouldn’t hang out with them outside of a medical/professional setting anyway

BTW, you can usually tell who will be like that when the are fellows

Most brilliant doctor I know doesn't care that I don't call him Doctor. This guy is actually a bit of a rock star in the Trauma Surgeon world and it always freaks other medical people out that I don't call him that. It's kind of become a gag between us. He's this highly respected medical professional and I am "oh that's Bob."

You can tell when they are residents.

They are the ones answering the phone as “this is doctor so and so”.
