My college roommate is now a Thoracic Surgeon. I make my kids call him "Dr. XXXX". He insists they call him by his first name...but unless he did so, they would refer to him as Dr.

One of my close friends is an MD. He doesn't do this but I make sure to introduce him as a nurse to keep him humble.

Unless he is my doctor … he or she gets called by their name. I call them doctor out of respect when I choose them to provide professional services and not otherwise.

Doctors are full of egos. All they are is another service provider. Mechanics are service providers.
I dont like calling doctors "doctor" inside the medical facility. They are people just like everyone else.

I’m an MD and I won’t even correct a patient who calls me Mr. when I’m clearly in my white coat and scrubs in their hospital room. I only ever correct people when it’s clear they’re being douche.

I know a medical doctor personally. He's over 70 years old and told me to call him by his first name. Nope, I call him Doc.
People with class do not bestow title upon themself. Respectful people show respect where due. In personal settings, if I talk to a retired Lt. Col. or Navy Cmdr. or above, I definitely address them by rank. For retired enlisted, I will do so for E8 and E9. If it is a Marine who retired as E7, I'll call him gunny. For a young doctor in a personal setting, I'd address them as doc. If it is a personal friend, then I'd just use their name.
If I meet someone and they introduce themself as Mr./Ms. etc. then I am thinking what a total cheese ball. Exception is when some elderly man or woman does it, then I just roll with it.

I do it all the time in my workplace and it doesn't make any difference to me.
