You left out the healthcare supply industry. I am sure there are other places that are stashing money but consumable lab and health treatment supplies are like a 20x markup over what they cost to make.

Go buy a bottle of turkey injector spice and you get a syringe with it to inject your turkey. Go get same syringe in from a healthcare supply company and it will be $25-$50. Just disposable sample cups and bottles are insane in cost.

Of course we are partly to blame because anything healtcare related requires product traceablity and quality assurance papers.

OLOL is a "non profit" LOL. They can buy as much land as they want and never have to pay property taxes. We have a brand new clinic built down the road owned by St Francis. They haven't opened it now for over a year and its for sale. Never occupied. Do they care? Nope. Don't have to pay property taxes. They bought North Monroe Hospital back in the day to eliminate competition and then let it sit and rot. Now condemned. Then sold it. When HCA owned it the parish received almost a million a year in property taxes, zero from St Francis.

You’re looking for one big villain holding the bag, but that ain’t quite how it works. The money gets shaved off in layers through insurance, hospitals, drug companies, billing departments, staffing agencies, consultants, middlemen, and a whole lot of suits who never touch a patient.

So yes, nurses can feel underpaid, hospitals can claim they’re struggling, insurance companies can show modest margins, and Americans can still be getting skinned alive. All of that can be true at the same time.

It’s a mess

CMS basically forces facilities to see their patients and then sets prices. They also require the fee for service billing system that everyone hates.

Under EMTALA hospitals have to treat patients - and then collect if they don’t have any money

Health insurance reimburse above the deductible and then forces hospitals to collect…with zero leverage.

Insurers have their networks that they use for leverage to drive pricing - don’t agree and your out of network.

Cost to expand to code, have equipment needed, medical supplies, is astronomical.

To protect from lawsuits you need complimentary services. Have an ER or OB? Need a surgical suite. Have an ER? Need a mental health outlet to deal with crazies.

Laundry, meals, maintenance, clinical and administrative staff to handle 1,000’s of employees.

No pays and bad debt are huge.

That said, they blow money on construction and other BS but it’s a total cluster of hobbled together systems and payment for services coming entirely from 3rd parties.

Neither do you. That’s why we should stick to the argument instead of pretending either one of us has a backstage pass. If you think what I said is wrong, point to the part that’s wrong. Simply declaring I have “limited knowledge” doesn’t move the ball an inch.

Same problems with throwing money at the school systems.

All the money gets spents by the administrative staff and higher ups

Lots of layers that add nothing to actual patient care. Bloated administration, closing and billing, insurance, etc. all expenses that are paid before the bottom line, keeping those margins down.

In a very short time, we are going to be in a situation where AI can look at our system and optimize it for lowest cost and best outcomes. It will be interesting when the capability is there to see how the current system pushes back against such realignments/redesigns.

Sadly, the last factor considered in the equation is the patient!
The current system needs to be gutted and rebuilt from the ground up.
