Speaking specifically about West Virginia, Kentucky, and Tennessee.

Every time i drive through I marvel at how pretty it is and then I stop to get gas or eat and im blown away at the type of people i see. Some of the areas I've passed through look like a third world country.

Why is it like that? When did that start?

Poor AF, Isolated, uneducated. It's like a Perfect Storm for trashiness.

A lot of their regional economy is based on coal mining, so there's that

Visit Scotland or Northern Ireland, or the borderlands of North England and Scotland, and it is the same story.
So, genetics.

How is it isolated? It’s not like the west where there is nothing for miles.

Pittsburgh, Asheville, Knoxville, Chattanooga, & Huntsville are all pretty dope cities with lots of cool smaller towns around them. Some of the really backwoods areas can be sketchy, but the entire area isn't what you're making it out to be.

Because the vast majority of the population has a high school education at best and their blue collar jobs are disappearing.

So, lack of education and money.

ETA: this is in regards to the small mountain areas of Appalachia, not the more metropolitan areas.

They are poor. The average income in some of the areas is 1/4 of the poverty line. The lack of understanding that a higher income is available elsewhere is generational.

Some of it is beautiful country though.

Scotch-Irish clans settled there after being run out of Ireland, while not being able to return to Scotland.

They kept their old clan family culture, brought over their stills, and the region is pretty difficult to travel into, so they were able to remain isolated.

My guess is the OP made several wrong turns off the highway and ended up in an area where he shouldn't have been with spotty cell phone service.
