While you are correct about calories in vs calories out in regards to losing weight...you've mistakenly conflated "health" with "weight" just because you are thin or fat does not mean you are healthy or unhealthy.

If a guy has good genetics and eats mcd every day, he'll still live a long time, but he won't be what he could have been if he'd have eaten better. Also people will conflate the mcd with his genetics because people are stupid.

The popular mantra that a calorie is a calorie is a calorie is correct from a physics standpoint as a measurement of energy, but not applicable to how organisms, in this case us humans, process foods. Just as you could say that gasoline and diesel both could contain an equivalent amount of energy measured in calories, try putting diesel in your gasoline engine and see how that works out for you.
A popular example is the intake of sugar via fruit, which also usually contains a high amount of fiber, versus a calorically equal amount of candy. You've consumed the exact same amount of calories, but the biological availability of those calories are significantly different.
Try cutting out added sugars entirely (which processed foods usually contain in abundance) for a month and you will absolutely melt pounds off even if you actually increase your total caloric intake by eating more.
I did this six months ago and went from 6'2.5" and 219 pounds to 190 pounds without changing my exercise regimen at all and eating more calories every day (I am constantly eating now).
For the sake of complete disclosure, when I say that I didn't change my exercise regimen at all, I didn't exercise before and I still don't. This is something I'm going to change because I now feel so much better and so much more energetic not carrying an extra 30 pounds around. I have to wait to get the okay from my doctor in a couple of weeks though because I'm an old frick.

I swing by the neighborhood mcdonalds for breakfast once every month or so. Usually see the same 3/4 old people there every time. Honestly you could probably do much worse for breakfast alone then mcdonalds.

I eat breakfast there about twice/year and always enjoy it. I feel like it would quickly get into the diminishing returns theory if I tried it more often.
