Even if trans whateverism is not by choice somehow, the miniscule % of people afflicted with the true mental illness don't deserve some special protection while they play make believe. The only mental illness you can't joke about or even slightly disagree with in public without real-life consequences is depression/schiz/bipolar. No one gives them any protection, yet they are technically just as, and often wayyyyy more sick. Removing trans from the mental illness book doesn't make it not a disease anymore, by the way.

at some point, the truth comes out and the cream rises to the top. These activists may bully and threaten their way into the outside world but if that is their only claim to fame, they will eventually pay a price.

I like to believe this but you see the same type of thinking at the top of our most "prestigious" institutions everyday.
