From what I've read, diet, exercise, and mental stimulation are all key.
Other things I've read but aren't doctor prescribed for mental decline:
Viagra - it helps bloodflow to the brain. (All the extra sex won't hurt either)
GLP-1's - help fight brain plauqes.
Personally, I'd hold off on the GLP 1's until it's widely available in generic pill form, and more vetting has been done on it.
Viagra? Go wild man.

I've gone through a lot of this when one doctor told me I was prediabetic and the next day a different doctor told me I had Diabetes 2 and prescribed five pills to take every day.
I went to the library and I got books and started reading. I stopped taking the pill cocktail at day 17.
I changed what I ate, I lost pounds, I kept reading. I saw my blood glucose drop. I kept reading and doing. My blood sugar hasn't been prediabetic since December. My A1C has been 4.1, 4.2.5.2 for the most recent three month interval tests.
Also read about fasting. One of the sharpest minded men my husband knows in his car club is 92 years old. My fasting was mentioned by my husband and the old gentleman started telling him how great fasting was and how he's been doing it for decades. That got my DH's attention more than my periodic fasting this year.

For the elderly 70 and over, about 50% of the medical treatment is pure quackery based on running the $ with mri's and maintenance visits.

In addition to the good advice in the thread, if you're in BR, look into what Dr. Jeff Keller is doing at Pennington. There may be trials or something more to learn from his local research.

I hate to break it to y'all, a friend is a neurologist, he told me that the brain isn't a muscle, exercising it does not make it stronger. The good news is on the pharmaceutical side, new drugs are proving to be very useful in delaying decline.

Read books, stay active, do puzzles and thinking activities. Learn history and something new. Take Lion’s Mane supplements. Supposed to help with that.
