But yes, there should be some kind of discretion, like with a therapist or a lawyer. They shouldn't have the ability to out their customers. If they do, they should be sued. They know that people giving them money want some level of secrecy. This is too much of a power imbalance that results in blackmail and manipulation. There has to be checks/balances..
