Because a bullet went through his shirt.
There was a video posted yesterday that was slowed down and went through like 4 or 5 frames and you can see his shirt jump around where his collarbone meets his neck. His shirt jumps from the impact, looks like it hit his collarbone and comes out his neck. Before all the bleeding you can see the bullet hole near the neck of his shirt clear as day.
