This is awful and tragic, and likely a failure on many levels. That being said, I've been in many an abdomen and/or pelvis that it is nearly impossible to tell what is what and the answer is identify a known point of reference with complete certainty and work toward the offending organ or structure from known to unknown and if you can't go any farther, get another set of hands/eyes that can or stop to fight again another day. You get a patient that has stage IV endometriosis, or has had pelvic inflammatory disease/tubo-ovarian abscess or prior colectomy for Crohn's disease or ruptured diverticulum, etc and it can make a complete mess of the anatomy.
I'm guessing this most likely had to be an endometriosis case and very likely in a woman that had already had an ovary-sparing hysterectomy, with severe anatomic architectural disruption. Even then one would expect with a foley catheter in and back filling the bladder, one would be able to delineate the bladder margins. Also you would think the surgeon would have tried to identify the ureter at the pelvic brim where it crosses over the common iliac and it can almost always be identified then open the peritoneum there and work downward, which can at least get you pointed toward where the ureter tunnels into the bladder trigone. If you are taking an ovary you should 100% of the time identify the course of the ureter as it crosses over the blood supply to the ovary (infundibulopelvic ligament) anyways in order to isolate that vascular pedicle, so dissecting out the course of the ureter almost surely would've helped here.
I can envision a situation with the anatomy so distorted how it could happen, but still of course should never happen to have a bladder injury to the degree that it cannot be reconstructed (excepting of course a procedure where it is removed due to an invasive malignancy which can be the case in advanced stage or recurrent cervix cancer). Awful story, hate to hear of such things.

In Louisiana, she would have been limited to $500K for everything - - all the hell she has and will go through. All the quality of life sold for $500K.... except future medical - - - and even then, all her future medical would be held by the State and she would have to go to her case manager hat in hand everytime she needed something.

They removed her bladder. Perhaps not 100% of it, but enough of it that repair was not possible.

How does this happen? When my mom had knee replacement, 3 different people came in and asked her to state why she was there, then one of them drew a big circle around her left knee to indicate which knee was being cut on, then everyone signed papers.
It is unfathomable to me that with all the checks and balances in place, an error like this could occur.
