I just walk off, and go do my thing. I manage my schedule like a professional, so I know when I can go do whatever I want.
I will add....i do absolutely everything possible to avoid having to take a dump at work. Like eat at the same places, and nothing abnormal. Take good morning dumps, before showering of course. Probably took less than 5 dumps at work last year.

Some people I don't tell anything. If it's someone with whom I generally discuss these kinds of matters I generally say "I'm going poo poo."

I just get in my truck and drive 5 min to my house and use my own bathroom without telling anyone.

I’m pretty sure people know I’m going to take a 20 minute shite every morning at work right after I have my coffee. Sometimes before I can even get half way through the coffee. I don’t have to get s hall pass though.
