I've done one Tri. It was at Bridgeland in NW Houston. The open water swim was in the lake there. Let me tell you, open water swims in Tris are mayhem. Imagine swimming in a frantic group of 50+ people who all go in the water in a random conglomeration group. So fast guys are behind slower guys. People slam into you, kick you in the face, swim over you, it's a brawl. I worked my way to the fringe and probably swam an extra several hundred yards just trying to keep from getting aquatrampled. I bet I swallowed a gallon of lake water. It was bananas.
I.did the Half Ironman in Austin one year. Had to swim Lake Austin I think it was called. The swim course was in a triangle with turn buoys set up at a few points. My first step was to get the hell away from people so I swam out towards the fringe. Took longer but only a couple of kicks in the head.

You’re describing a mass start, IM Texas was a rolling start or in waves. Not saying that eliminates contact, but it’s not the “washing machine” of the old days. Also, nothing embodies FAFO like a slow swimmer lining up at the front of a mass start swim/triathlon.

Not gonna lie, I swam in a tri and had a panic attack out in the open water. Came out of nowhere and completely unexplained. Fortunately there were lifeguards in kayaks.
Still to this day, I can't explain what happened. Like it was some weird claustrophobia taking me over.
To add, I was an adequate swimmer. Wasn't even a long swim. Like 400 yards.

Did you do any open water swims prior to the race?

My wife did the alcatraz race that year elllingers dad died. Only time i have worried about her safety in a race. Zero visibility, huge currents and 54 degree water that day. I believe he had a heart attack soon after hitting the water.

Not being able to warm up in the water is a big reason Ive never done Alcatraz. I mean, I get they can't let people off the ferry and I have raced in really cold water before (58) but even with the small chance, I just don't see me enjoying jumping in the bay. I need to get myself ready for water that cold.

No,only laps in an Olympic size pool.
I think that contributed to it. I could not touch the bottom, etc.

Didn’t a feller die of heart attack running a marathon recently too. Zero reason to push your body to extremes.
you're focusing on rare outliers baw.
pushing to extremes has risks but not pushing your body to extremes has higher ones. People who push training/exercise to extremes are more likely live longer and better.
