Chances are someone mixed their cleaning agents and accidentally created phosgene gas.

Clean with bleach. Clean with ammonia. Why am I keeping these separate when I can be more efficient?

Bleach and ammonia make mustard gas, not phosgene

Don't worry, everyone. I'm checking in safe. I know you're all worried about me.

Someone didn't watch their safety videos.

In all seriousness that is awful that someone died.

Neither of these, it puts off chlorine gas...mustard gas has a sulfur in it. Neither bleach nor ammonia has any sulfur.

Or, someone did a little googling and decided that one of their co-workers sucked at life and it was time for pay back.

NH3 + NaOCl -> NH2Cl + NaOH

We get it. You can all Google.

I wish people still had to learn chemistry the old fashioned way... By blowing shite up.

Brake Fluid and bleach makes awesome reacting targets for shooting... Just don't mix them.
