My S&W .45 can have a dry trigger pull to chamber a round instead of using the slide.. so two pulls equals one shot on the first round.. I may not be explaining the mechanics right but that could be what he was referencing

If you'll zoom in you'll notice this holds a .45. Each time the trigger is pulled, the next round rotates into the "chambered" position.

He tells the story far more in depth in his book.

About how he entered the bar, the woman he flirted with, what she was wearing, how it all ended with him in a mental hospital.
