This plant didn't have a safety department for a long time lol they want it done now and they want it done fast. Very little redundancy in this plant, when something plugs dollars start disappearing. The plant is so far in debt that theres a ton of pressure to get these jobs done quickly

MSHA is there so often, they didn’t really need one.

We had a couple guys hire on that came from there. All said it’s an absolute shite hole, but you could make as much money as you wanted, they allow you to work everyday and just hold a broom for OT if you want.

Loto done "correctly", line was plugged... plug blew out
Obviously not done correctly if line was still pressurized. To do proper LOTO you would go back further in order to vent the pressure off safely before working on the line.
OSHA gonna have a field day with this one.

Assuming you mean an engineered plug… that’s a ballsy move. Not sure I’d approve work downstream of one of those in caustic service.

Wasn't a paywall for me and, no, I do not subscribe to anything from the advocate.

They will be closed soon. I don’t know why companies are still performing work / selling product without receiving payments from atalco. They will probably file for bankruptcy soon if I had to guess

Its not an OSHA plant for one, if the pipe was plugged, depending on where the bleeder is, you could correctly loto and verify no energy at the bleeder but still have this happen

They have been on the brink of bankruptcy for years it seems. All the people that work there are so used to it they are offended when you ask for payments on very late invoices.
