I taught my kids to question authority a good bit at a young age, seemed like a good idea at the time. Now that they are teens, I’m the authority they question and it seems like a terrible idea.

Youngest actually. I don't teach them to question authority but I do teach them to think for themselves and to respectfully challenge things they disagree with.

If not... demand they hand you a copy of it.... when they can’t.... demand they offer an apology and exonerate you child.
This has to be the first step of challenging the results. You need to see the document in the handbook that spells out the "proper" way to wear a mask. You also need to document when you, or your child, notices teachers/admins not following these rules and ask what is the discipline for the staff setting bad examples and showing kids the "improper" way to wear the masks.

Sounds like you were smarter than me on that matter. You seem bothered by this punishment so my first feeling would be to say address it with the teacher in a respectful manner. Good luck with navigating the ever frustrating school system.

As a principal in a public elementary school, I push back against teachers writing up kids for not wearing masks. We have to be aware of how hard it is for students to do this all day!
And don't let people say you are excusing your child's behavior like other parents. Couldn't I tell you all some stories where parents actually excuse their children's behavior. I actually had a parent try to excuse their kid for choking another student!

Ask the teacher if he was properly trained in use of the mask before handing it out.
As an employer, it is their responsibility that employees (students) have proper training for PPE.
Ask her also if she is certified to train the students on the proper use of the students PPE.

I think people really don't understand what these students are going thru! They have lost so much in their short years of their youth.
Playdates, organized sports, running on the playground during recess with out a mask, trying to pay attention in class with a mask on your face! It is just horrible what we are putting these kids thru and believe me, I have cried numerous tears over it!
