if i had to guess the floors caved in. columns are built first then the span across the horizontal beam with the floor. in a steel beam building, the span bar joists then put the decking down, the pour a 3-6" concrete floor. on a concrete building they form and pout the columns, then deck the floors and support to the deck below with "trees" while the pour the floor. once it has set up they remove the wood deck and the "trees"

my guess is the joist were over loaded and 5 floor of joist and decking collapsed
