Medical school in India is dogshit compared to the U.S.
The AMA needs to focus on building homegrown native doctors instead of importing a bunch of IMGs from countries where the quality of schooling is subpar compared to the West

Typically, everyone is sued. Doctors, circulating nurses, charge nurses, the hospital, etc.
There are multiple pairs of eyes that see what's going on during surgery. Good lawyers will pit people against each other to get at the truth.
Ultimately, the hospital, Doctors and nurses insurance pay.

Though it is not specified anywhere that I can find, it sounds like, from descriptions of the echocardiography, the girl's pulmonary valve was being replaced. Sometimes that is done using a prosthetic aortic valve as replacement. Regardless, it would usually be done with a pig or human cadaver valve, and it is difficult to envision how one of those could be placed "upside down" by even a semicompetent surgeon.
Thank goodness it sounds like she was eventually transferred, diagnosed appropriately, re-operated on, and survived.

However, in this case, given the magnitude of mistake, I wonder if this fellow was passed through those programs with knowledge that he was not up to standard. For obvious reasons, I would hope at some point in the near future that possibility would be thoroughly investigated.
