I think it's depends on where you're watching it. Local cox cable is still local I think whereas YouTube TV or something is national

The local aspect comes from hardware and ad-insertion systems physically owned and operated by local cable and satellite companies at their regional hub or headend sites. This supersedes the national feed during that time slot.
If you're watching through something like Hulu, they don't have that so you get the raw feed which goes through the different regions.
