Brooks Brothers. The quality isn't what it used to be after their Chapter 11 but it's still better than anything else around $100/shirt.

Depends on what fits at the time. I find brands are ever changing and most brands constantly change sizes to fit fatter people.
I recently got some made to measure shirts and I might never got back to off the rack. More expensive obviously but they’ve held up well this spring and summer.
