Def one of them, so is Rupert Murdoch buying Myspace. Motorola doubling down on flip phones instead of moving toward smartphones in 2007 was also a pretty bad decision.

Joe Green, he was Zuckerbergs's Harvard roommate who co-created a social network website with Zuckerberg that would later become Facebook. He choose to stay in school instead of dropping out to help Zuckerberg start Facenook. Green's father was a professor that said finishing his education was more important.

IBM would have bungled DOS by keeping it from progressing. They would have "fixed" problems by throwing tons of cheap labor at it and ended up with a painful compatibility matrix that stifles innovation.
Apple and some other Linux derivatives would have beat their pants off and they'd be even worse off because that's less x86 hardware IBM would have sold.

I'd argue James Monaghan is up there. From the wiki James and his brother Tom took over a small pizza chain for $1400 (this was in 1960). The business did well enough that it cut into James' hours as a full-time postman, so he traded his half to Tom in exchange for a VW Beetle.
James thus kept his job delivering mail while his brother grew the business into Domino's Pizza.
Also, supposedly one of the earliest uses of bitcoin involved buying a pizza for an ungodly number of BTC that would be worth many millions today.
