personally, I've never worked for a large corporation or the government where anyone actually keeps up with sick days.
Working for a small business means that whether or not I can take off all depends on what's scheduled that day. There are days that I could be on death's doorstep and have to go in, and there are days where I just don't feel 100% and I call in.
Like most thing in life, it has it's advantages & disadvantages

So if a company just gives employees a bucket of PTO, and the person uses it all in the first part of the year. What happens when they legitimately get sick later in the year? They get fired if they get sick? Dont get me wrong, fk that guy, but is it not a liability to have a guy working that has no PTO with months to go?
I struggle with pto, personal days and sick days, we have tried multiple different systems, and I've come to the conclusion, people just suck. Theres no system that wont get exploited; you just cant hesitate to fire people. Everytime we try to make system more lenient, it seems to backfire.
Go out of your way to reward the employees that respect the system, cut ties with the bad seeds as fast as possible, and the rest will respect you more and start to police themselves as you say. That is my current line of thinking.

I want to say I have read that employees at companies like that take less time off on average. I guess no one wants to be the one that abuses it?

My company, a healthcare corporation btw, frowns upon taking sick days. Like I wrote, if it's not scheduled, it's frowned upon. You can't schedule getting sick, so frick them. Personally, I think they're the shitty ones.

Thanks. I have surgery this Tuesday to get a laminotomy procedure done in the hopes of relieving some pain. I have a shallow fluid canal and that is the main cause of my nerves getting pinched. Wouldn’t wish the pain I’ve felt on my worst enemy. Good luck in your journey as well

They'll get theirs in due time.
They'll get sick for real and not have any PTO left, so they'll take a hit on their next paycheck. And this is the type of person who has no savings.

I had this at my last job. AMAZING.
Hardly ever took sick days because you can just schedule a day off if you want one or just need a day to get personal business shite done.
You still were responsible for getting your shite done and making sure your deadlines were covered.
Most people ended up taking 4-5 weeks a year. 2-3 weeks from Jan-Oct and then a few days during Thanksgiving Week and a week or two at end of year.

For my company you only get 90% of your daily rate on sick days.

I get 10 sick hours per month and will get 14 once I reach 10 years and it all carries until you get paid out. I take sick time pretty much whenever I want but I'm not dumb about it.
You only get 7 days a year though. How close of attention are you paying to your coworkers that you know they're taking a 3 day weekend 1 every 2 months or so?

Not to mention, if they are in day care, you will find out really quick.
Its quite easy to use sick days with kids. If your child is sick and you need to take off of work as well.
