Tragic story and I grieve for the OP, but I'm pretty sure there's no hospital anywhere that would send her home if "she couldn't wake up from anesthesia."

Yeah, and they treated her with laxatives. Still absolutely tragic, but it was a PE in all likelihood. I am not sure an autopsy is needed.

Eh.... My neighbor is super sensitive to anesthesia. Big lady.

The last few times they kept her overnight and then sent her home before they should have.

Is 3 the standard dose for a patient unable to wake from general anesthesia?

I imagine it's poorly worded.

I would give them some slack if this is true. Pretty hard to think straight.

I would lawyer up. If a patient dies within 24 hrs of a hospital admission then an autopsy is required (or at least it used to be). A physician can't refuse an autopsy, especially if he/she is possibly responsible. I wonder if the sister meant the orthopedic surgeon refused or if a CMO refused. Regardless, an autopsy should be done by the hospital.

That's how I took it. Not that she was sent home still under, but that she had difficulty coming out of anesthesia signaling perhaps a problem in conjunction with the low BP.

All hospitals are not created equally. I have no idea where she received treatment but a lot of hospitals are staffed by incompetent people.

I am 100% sure. In times of stress people communicate poorly and the meanings aren't always clear but the OP and the sister have put too much out there to be scamming. Its real.
