I don't know about everyone else, but my "inner thoughts" sound a lot like this on a daily basis. They're just saying them out loud-ish
It's more likely that they live alone and don't have others to talk to, so they've developed this habit they don't realize. I find myself talking out loud all the time when I'm away from my family or home alone for lengths of time, this is just that on a more permanent level

I say shite to myself that would start a fight if someone else said it to me. Depression is a hell of a drug.

I have conversations with myself all the time. When I think people can’t hear, I do it out loud. It’s usually when I’m trying to work through a situation by taking both sides of a debate and going back and forth. I’ve done it my entire life, and I’ve turned out fine. (And I’ve turned out fine as well.)

Could be psychosis but if not harming anyone or self...I'd leave it alone, especially if there is no history of violence. With that said, if paranoia is present - help is warranted

A psychiatrist typically writes prescriptions for your issue(s). A psychologist listens to your concerns and works through solutions with you.
I'm not so sure you family member needs help. Perhaps he feels that talking to himself is the only way he can carry on an intelligent conversation.

30-50% of people have inner monologues. I am one of them. Some of those people also have external dialogues. I’ve been guilty of that too, mostly when no one is home. I talk to the dog all the time but really it’s me just talking to myself. It actually helps with clearing out the thoughts rattling around my brain.
