I'll have to think about the most unsettling or disturbing, but definitely the most depressing movie I ever saw is House of Sand and Fog.

The Fog. It was a decent syfi/horror movie to the end and then WTF! I had emotions.

Another vote for Requiem for a Dream.
Brilliant movie. But a once in a life time film. It’s a difficult, draining experience.

I still have no idea what was going on but the entire film has this mood that makes you feel constantly uneasy and creeped out the whole time.
You want to stop watching it because you feel like the movie is watching you. So creepy...and it's about the internet.
