It's not a twist. I think he is referring to the sudden death at the end (saving spoilers).

Is there any other animated disney flick that has a sad ending? I can't think of one (I guess pochahontas could count.)
Bambi
The only thing that keeps me from crying is knowing how good bambi's mom must have tasted on the grill...yum

Usual Suspects has to be on any top five list. It is infamous for it's twist.

some not mentioned:
A Tale of Two Sisters
Friday the 13th
Planet of the Apes
The Ring
And the winner is:
Empire Strikes Back (seriously, nobody has mentioned this?)

shawshank redemption was by far the most shocking to me

1. The Sixth Sense
2. The Usual Suspects
3. Fight Club
4. Empire Strikes Back
5. The Ring
After reading the list, I have to give honorable mentions to Oldboy and Presumed Innocent.
I guess I was just that intuitive (not really), but Primal Fear didn't sneak up on me at all.
