But with that said, I'm not going to not prescribe it for someone if they ask for it.
If they don't have a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2 (MEN2), which are the two contraindications, then I will write for it for anyone who asks. Who am I to stand in someone's way to achieve weight loss when losing weight will improve a long list of other chronic medical problems.

It does me no good to refuse it to anyone.
Last time the Mounjaro reps stopped by, they told me hopefully by June they will get FDA approval for obesity and then everyone can have access to Mounjaro.

A few of us in the crypto/penny discord group have had incredible success with a carivore + fruit diet. The fat melts off you and you eat like a king. My 6 pack wasn't this good in college. I'm not kidding.

Completely agree on BMI. I have chimed in on multiple threads on BMI stating that a simple navel measurement will suffice. Not a fan of it at all. Do you have any data yet with anyone who has been on it, got off of it, and did they maintain/gain weight? Would you consider this something that you would need to be on indefinitely or take as needed?

keeping it off really comes down to....did they use the medicine to learn healthy habits or did they just use it as a crutch. from the dozen or so people i have known who got on and then got off....its pretty much 50/50 on those that kept it off.
its really comes down to learning to really building healthy habits and keeping them
for the record, nothing wrogn with staying on it if insurance covers it. lots of health benifits with it.

I have yet to see anyone back who has stopped it. All my patients are still on it.

I tell all of them that this is the same as any other diet. You have to make the lifestyle changes for the long term. This will suppress your appetite, so the minute you get off you all likely to resort to old habits. Hopefully while you are on the shots, you learn to increase your exercise and that you don't need that shite food you are used to eating.

is there any hesitancy to prescribe these to older (55+), non obese patients that are mostly using it to drop manageable weight?
the studies showing a decrease in up to 40% lean mass with these weight loss drugs is very concerning to me, especially in older people, considering how difficult it is to just maintain lean mass, much less build it back up as we age

i mean it is concerning, but if you are carrying excess bodyfat that is a much bigger concern.
99% of those seniors arent lifting anyways so chances of maintianing lean muscle is slim anyways
and the 40% is mainly because even on these drugs, peopel are scared to resistance train.
remember less than 10% of the population actually resistance trains 2x or more per week, in any form.

True. For obese patients, I'm all about it. My thoughts are more for the "vanity" patients or patients with more manageable excess weight, like 30 lbs or so.

Right.
Which, IMO, needs to be the push. And even moreso if they are taking these drugs.
I just consider this to be more of a significant side effect that many are not considering.

It’s not vanity it’s more a mental health thing
I identify as a 6 pack haver but I’ve tried everything and still don’t have one. Who are you to say I can’t be the person I’m supposed to be?
