Pitcher striking out 20 in a game. Only been done 6 times, by 4 pitchers. And on 2 of those occasions, the pitcher threw more than 9 innings.

It's a perfect game, hands down. When that many games have occurred over the last century, and there have only been 21 or 22 (I still count the one the ump blew a few years ago), it's easily the lowest odds to occur per game. I think that makes it the hardest pretty handily.

Both of those involve some level of luck, hitting for the cycle requires more skill though.
Most difficult thing to do in sports at professional level
1. Hitting a Baseball
2. Consistent Ball Striking in Golf
3. Stopping a Penalty Kick
