Unless it was a used gun someone was bringing into the store i am not sure how else it could have had a live round in the chamber. I have never been into a gun store where they had the ammo in the same vicinity of the firearms nor have any of them ever showed me how to operate the weapon with live ammunition.

I'm honestly surprised I haven't been shot by a customer yet.
I can't tell you how many times a customer has handed me a firearm saying that they unloaded it I want to check it lo and behold around falls out .
And no a sign on the door does not help .

How many mistakes in this scenario?
Several have already been mentioned but one more huge no no on a rimfire is pulling the trigger on an empty chamber (well it was supposed to be empty at least)

This happens more than we sometimes see.
A few years ago a customer walked into Precision with an "unloaded" Ruger LCP and shot though a display case. Wounded another Ruger pistol. Customer had to pay for the glass case and the wounded Ruger.
Here's a video of a cop shooting his finger off in a gun store. (And for the children -- yes, we discuss this in my classes).

That gun may have been loaded for weeks. Somehow, one person is responsible for it, I would assume they'd never truly find out who though. Obviously, this idiot is responsible for his own actions as well for not clearing the firearm first and then showing the customer that the firearm is cleared (so as not to get shot by the customer who could be a lunatic), but I would think that he became complacent and assumed his coworkers were practicing safe and responsible procedures and that the firearm was never loaded in the first place.

This. My favorite was with some college kid and an A400. Brings it in for some feeding problem. I charge it just to be safe and a shell flies out. Handed it back to him and said "nuh uh. Go somewhere else." To this day, I don't know what he said - it was so fast and so slurred. The sheer embarrassment and panic sent his language skills out the window. It's like he was speaking in cursive

I worked up front there years ago. I was trained on the different types of firearm and how to ensure they were unloaded and locked before they made it further in the store. There is a clearing station where you are supposed to "clear" the gun also in this process.
Not sure if that training or process is still in place, but I'm taking a wild guess in assuming that it's not.
