The funny thing about this is that the movie and the book made the point that Billy was not a true transexual, he just hated himself and the skin that he was born in. He had sex with men (Benjamin Raspial) which makes him more of a homosexual homocidal maniac.

I think most Americans would be shocked at how mentally sick society truly is and conversely how quickly society would return to a very healthy and vibrant society if the hundreds of thousand if not millions of mentally disturbed individuals were rounded up for mental health treatment.
