re: Would you refer to this person as a “doctor” outside of an academic setting?

I think the reason everyone called him doctor was because he was at the pinnacle of academic hierarchy, was older, and was very distinguished in his looks and his manner. He wasn't some young slapdick.
This, and he probably didn’t insist upon everyone calling him that. If someone he didn’t know called him Mr. Jones (and me) in a casual/non-formal setting he probably didn’t care

This is the only doctor you need to feel just right.

My mom has a PhD in education.... She sends her grandkids birthday cards... The return address she lists herself as Dr. She signs her checks as Dr. When she got her Dr. She for herself a new email address..Dr. Name @gmail , she has had the same one for 20 years prior to that. She now wants all emails i send to her to go to that one. I refuse and send to the other one.
She Is EXACTLY the kind of person you think she is.
Love my mom, can't stand the way she acts about things.

I had an elementary school principle like that.

My wife (no pics) has a PhD and only goes by Dr in her professional setting. Definitely does not use it outside of work.
Come on now. You know she makes you call her Dr. when you are having some sexy time.

People with a PhD who insist on being called "Doctor" outside of professional settings are attention whores.
People who insist on others using any professional title outside of a professional setting are attention whores.
The MD’s I know wouldn’t introduce themselves as “Dr. [name]” at a tailgate, or a party, or whatever. That would be pretty weird for anyone who isn’t a raging narcissist.
ETA: I think PhD’s are the ones usually making a big deal about this because a lot of them have some sort of weird inferiority complex.

Whatever it takes to make them feel better about their godawful life choices.

Just a little adjustment for this thread… remember, not all academic doctors are professors. The title professor comes with being tenured or being on that path. So it’s sort of a rank above “doctor.” So alternately referring to an academic doctor as “professor” would be in some cases inaccurate.
I think it’s ludicrous to suggest that a Ph.D. should “never” be called doctor. In an academic setting, that is what’s appropriate. Outside of that setting, there’s no need for it. Most academics, save from a few screwballs like the example in the OP, see it this way.

If someone says “Help, we need a doctor” and you don’t go to them we don’t need to call you doctor.

People like this idiot are why I don't flaunt my PhD everywhere I go.
So many idiots can get a meaningless PhD. This is another one.

My grandfather has his PHD in Chemistry and everyone refers to him as Dr.

I say we go the other way. Outside of the medical office, you are Bill/Tom/Steve.

You shouldn’t be addressed as Dr unless you actually practice some form of medicine
Posted on 3/31/22 at 7:54 am to member12
I’m okay with this as long as she returns the favor and starts addressing her fellow graduate degree holders as “master” too.

My wife and I are both MD’s.
We have never asked someone to call us Dr. unless we were in a clinic or hospital setting.
We certainly have never put Dr. on a credit card or any other application.
I don’t know any of our medical school colleagues who have done it either.
Anytime someone introduces themselves to me as “Dr. ____” then I assume they are in academics and are deeply insecure.

Even if you are a medical doctor, I’m not calling you Dr at grocery stores, church, airport, etc.

frick no. Roast her arse. If we're not in a clinical setting, hospital, or talking about a medical condition that involves my family, then I don't address my wife's colleagues as doctor.
frick that lady!
