What's the worst you've ever messed up at work?

I forgot about a big crane I was supposed to send out on an important time sensitive job. Customer called and was pissed but I took responsibility for it they kept doing business with us. My manager wasn't too happy though.

Lost $200k on a project that was a shitstorm from the get go. Estimator overpromised on duration, and started us off in a bad spot. I fired the supervisor halfway through for constantly lying on the progress report, and battled weather every single day seemed like.

Opened up an email that turned out to be a trojan horse. Whole server got hacked and the information was held for ransom. My company refused to pay and our client's info got leaked. Apparently it led to quite a few divorces due to the nature of the services on our website. After a down year financially though we are finally starting to see a rise again. Although my emails are now vetted before being sent through to me.

Back when I was younger and cutting yards still. My boss who was also my friend dropped me off at a job. An obscene amount of torpedo grass. I was out the night before at friar tucks on feret. I'm am hung over like shite from that cheap bourbon they had. By mid afternoon I was in real bad shape. I ended up puking in this lady's front yard. The mail man didn't even stop to see if I was okay.

had this girl in the bank i worked in who worked downstairs.
she would bring her panties into the guys bathroom upstairs. it was just a toilet and sink and a storage cabinet, but each sex had their own. she'd hide them behind the cleaning supplies.
i would jerk it and release hell into her panties and then tell her i'm done.
she would tell me it was running down her legs. then go into these fantasy things, which with i'd reply. ended up being a pretty freaky relationship for a while.....until one day HR called and said that we were using company email inappropriately. (i was 25 and this was in 1997 so it was new to us).
lesson learned: you can blow your load in the hot secretary's panties during lunch in the mens room. just don't discuss it in email on a work computer.

One time I got stuck with an incompetent estimator on a big project with an a-hole for a boss. He fired me as a scapegoat for bad weather that was holding us back, but I got the last laugh when I fricked his wife.

I got caught in a situation where I didn't do anything wrong, but got the heat for it anyhow.
There was a meeting on a Thursday where it was decided that our organization needed some new web site content to go live by Monday. That's on me. The problem was that I was never told about the need for content, or the meeting.
So, Monday rolls around, there's no content. I get blamed for it. I tell them no one told me about what was needed. They don't believe me. It ended up being a big ol' nail in the coffin for me, which I should have seen coming anyway. After all, they had a meeting without me. That's usually a sure sign.

Not me, girl I worked with though emailed medical laboratory test results that were intended for the AFCENT Command Chief to the wrong person so she had to explain that to him:
Not supposed to transmit results like that period, but we were making an exception for this higher ranking guy, and she fricked it hard
Side story, logistical E-6 at our aeromedical evacuation unit (smaller specialized unit)would buy supplies that were requested, and not I guess on his government purchase card. Then return the items and get gift cards back. Sold the gift cards online. Stole night vision goggles and fun shite like that too, didn't get caught at all but he pissed off his wife one night and she turned him in. He became E-1 and played in the brig for a while

Sent an inappropriate text to some of my coworkers making fun of my boss. She got a hold of it and called me in. Wasn't too nice but nothing happened to me and she is now gone and I am still here so it worked out.

No I fired him for telling the client we'll be finished with this by XX date, and then XX date would pass and boom not even close. Reporting less than exact manpower daily so he could pass the buck to the crew being understaffed. However he forgot they had gate logs I pulled showing the full crew daily. Weather came in month three of our one month duration project.

I forgot to put the cover on a thermal exhaust port. It ended up being a major mistake.
