By 2018 fast food restaurant standards, they are:

Not as clean as some
Not as cheap as they used to be
Inferior product quality
Inferior employees compared to some other chains
Slower than they used to be

Are their fries the only thing keeping them at the top of the food chain (pun intended)?

The name.

Seriously. People are familiar with the name. When people are hungry, don't really know what they want, but wants to grab something fast.. It's almost automatic for a lot of people to say to themselves "I guess I just go grab something at McDonalds".

Part of the reason is because they are everywhere.

You can't sell something in which the machine to make it, is always broke.

Ummm, have you actually researched how profitable they are and what kind of real estate holdings they have?

Believe it or not, there are rural towns that don't have the almighty Chic Fil A, Wendy's or Whataburger. Or Canes, or you get the picture. But what they will have is a McDonald's.

Fast food is about convenience, so store locations matter a lot. Since McD's was the first fast food chain to blow up, they got first choice on locations.
