Texas teen electrocutes herself while using cell phone in the bathtub

It amazes me how often I see non-GFI outlets in bathrooms.

why would there be an outlet close to a bathtub? aren't there codes that don't allow that for this very reason?

Yes, but idiot-proofing electricity is pretty much impossible.
Wouldn't be surprised to learn that an extension cord was involved because "those dang darn stupid electricians put the outlet too damn far from the dang tub."

I'm actually extremely surprised that this doesn't happen more often. I've seen and heard of several people using plugged in phones while in bathtubs or hot tubs (usually drunk and their phones are dead after a night out). Reliance on GFI saving you isn't the way to go. Not to mention older homes that haven't been renovated often don't have them. Tragic story. Sorry for her family.

USB is 5v IIRC. She must have been in the act of plugging or unplugging the cube.

I've always assumed this as well, which is why I didn't lose my shite when I've seen people with their phones plugged in while in pools and baths. Apparently it's enough to kill you. Good on her parents for using their tragedy to help others out.
