A GFCI doesn't care about the ground circuit. It doesn't see it nor does it measure current through the ground conductor. A GFCI operates on differential only. When hot leg current is the same as neutral leg (leg = wire) the EM radiation cancels itself out and the GFCI sensor measures nothing.
If the current flowing through the hot leg varies at all from the current flowing through the neutral, the two currents no longer cancel out each others EM radiation and then current is induced in the GFCI monitoring circuit which then sets off the GFCI protection.

She died because water came in contact with 120V in the extension cord and she was in contact with the water.
I didn't call it exactly right but she died from 120V. The phone had nothing to do with this death. This was the equivalent of dropping a toaster or hair dryer into the tub.

Here's what I bet happened. The iphone cable got wet and brushed up against the frayed part of the extension cord while her hand or part of her skin touched the wet iphone cable.

The frayed cable resulted in a sketchy charge. Not realizing the frayed cable was the problem, she reached over to make sure the converter was plugged into the extension cord. Pop.
