One thing that might be a bit more believable is if the charger had a really short cord so the kid used an extension cord to have it plugged in. The extension cord was long enough such that the charger itself, not just the phone, fell into the water. This would introduce 120V directly to the water and would be much more hazardous/dangerous.

Not at 5 volts DC. Contrary to what people say, it is not the current or the voltage that kills you, it is the combination of the 2. A common way to think about electricity flow is to think of water flow as analogous. Voltage is like pressure, current like flow. so a pump may be capable of providing a certain flow and pressure but that pressure has to be enough to push the water where it needs to go.
I suspect it was the AC that did it (extension cord, as suggested by another) and I suspect it was a small 2 conductor extension cord so the GFCI would not come into play due to no ground circuit being present.
Purely speculation on my part.

You sure? The article indicated that she touched a frayed part of the cord, not that the cord or charger got into the water.
Either way, that's crazy / chilling that this was the last thing text she sent. Do they not teach electrical safety in elementary / middle school any more?
