Idk. Ive seen rollover MVAs where the person walks away without a scratch.
The most important thing is a good physical exam which is a lost art. Nowadays they rely on everything but that to make a diagnosis,

There is no threshold. Most health systems have shifted to value based care. And not many ER docs, in my experience, are solo. Most are employed by the hospital or health system and they carry and pay for their malpractice insurance.

Would you agree that in theory, this would make the doc less likely to be concerned to the point of avoiding broken bones?

If they did an ultrasound, gallstones are pretty easy to see most of the time unless the GB was contracted. Strange they would’ve missed this bc it’s one of the most common reasons for abdominal pain and visits to the ED so they’re actively looking for GB problems. Emergency medicine is tricky. Lots of BS to sift through with “interesting” patients constantly.

I use to work in an ER for a while when I was younger, I guess it depends on the hospital but depending on what your chief complain is that you cam to the ED with, they will order certain labs or scans aimed to addressed or cover the basics of what they came in for. If you came in for one thing but were dealing with a completely separate issue I am not sure how its their fault

There doesn’t have to really be “fault” per say. Just “standard of care” not being met. In this example, standard of care was not met and they should probably just pay for the one ER visit.
I say this as a broker who is usually arguing with the attorneys on behalf of my client.

My wording was bad in the last post but this was along the lines of what I was referring to.
Usually if the patient continued to have problems or their chief complain wasn't addressed we tell them to come back in and we will take care of them

30 mph bicycle wreck. Lost control on a downhill and got run over by another biker. Ambulance called and brought to ER. Main complaints were nausea (clammy) and the shoulder. The bike helmet has multiple dents and cracks. Road rash down most of the back. At the ER, ice placed on shoulder. X ray taken. No break found. 3 scripts written 600 mg ibuprofen, muscle relaxer, and opioid. Discharged with a sling after an hour and a half with instructions to see primary doc in a few days.
At home, shoulder complaints continue, lightheaded and paleness if standing too long. Clammy feeling remains. Go back to ER after 6 hours. X Rays faxes over. Same x Ray shows fracture. More tests ordered.
CT scanned ordered during second visit now too.
About all I know.
