I may be wrong, but I don't think many people are calling up their dealer asking, "bruh you got any of that fentanyl?". Distributors and/or dealers are cutting powder heroin with fentanyl, as it is much cheaper and much stronger.

From what I read, Acrylfentanyl binds a little more tightly to the receptor than fentanyl, but can still be displaced by narcan. If the person administering the narcan knows what they're doing, that shouldn't make much difference because you never know exactly how much narcan is needed to reverse an overdose. You keep giving more until it works and repeat as necessary if the narcan wears off, preferably with an understanding of how to manually ventilate if you just can't reverse the overdose with higher doses of narcan.
If the narcan is being administered by someone without experience in dealing with that situation and without enough drug on hand, then there's a higher risk for problems no matter what narcotic was used.

The issue is the volume/weight of fentanyl and especially carfentanil is so small compared to the amount of heroin and cutting agents you need to mix it with it requires very precise measurements and mixing equipment most dealers and small-time distributors just don't have.
I'm generally sympathetic to opioid addicts, while I am unaware of any personally I know a lot of alcoholics and the main differentiator I see outside the legality issues is many of the opioid addicts started in treatment of a legitimate medical condition.

Bingo. Wrap that in with the fact that people are seeking cheaper alternatives to treat chronic pain than what the absolutely megafricked pharmaceutical/medical/insurance industry pushes and you've got a really bad situation. But Darwinism, amirite?

You can all be rest assured that every holier than thou motherfricker in this thread has someone they love deeply who is addicted to Opiates and they don’t know. They would be shocked to know. They would ugly cry like girls if that person was found dead and alone somewhere.
