Not just the head of the agency, the person whose responsibility it was to collect and/or pay the taxes can be held personally liable. If they paid other bills ahead of their trust fund taxes, they can be held liable and personally responsible.

Yup. Trust fund civil penalty.
I've seen it used a couple of times. However, I've only seen it used in situations were the owner was involved in the business. Could a civil service employee be held liable here? Would be interesting.
Also, given the amounts and time periods involved, this is likely not the actual payroll tax due, but rather, late interest and penalties. My guess is the S&WB was consistently tardy in remitting their payroll tax deposits, and these late deposit penalties have added up. Not that it's much better... but it's more likely that this was just tardy payments as opposed to money being stolen / misappropriated.
IRS doesn't play around with payroll tax.
But in any event, unless the IRS decides to waive the penalties, this is more cost being borne by the taxpayers.

Less than zero, unless there is something out that there is much bigger.
If the S&WB ignored this, I could see the IRS levy bank accounts, though.

I got similar treatment.
Luckily, I'm just as petty, so they had to change all their cellphone numbers in the early 2010's.
If the property hadn't sold soon after, I'd still be having those frickers called 264 times a week at all hours of the night.
By now, it'd be like, "can you hand your gf the phone, bc she hasn't been answering her work line, cell, home, or returning the messages that I leave her momma."
