If you’re asking why does it exist as a street drug, Some street drug manufacturers will add it to occasional batches for publicity. If the news is blasting that local Heroin users are overdosing from an extremely strong batch of heroin, stupid users will want to try the “good stuff”.
It’s fricked up but they are willing to sacrifice a few junkies to get more junkies in the buying mood.

It's an extremely powerful synthetic opioid. It's a godsend for people in extreme pain due to things like cancer or late stage illnesses. Micrograms of the stuff kill their pain.

This is laughably wrong. Activating opioid receptors decreases the transmission of pain signals to the brain. It’s not just the high that decreases pain.
