Better question is if you have a friend that is a doctor - what do you refer to them as in their office when speaking with the staff?

When speaking with their staff I’ll refer to the doctor as Dr, but when talking to my friend I will just call them their first name.
Now, my wife is a Dr. and I just refer to her as her first name when speaking to her office staff.

IMO, start formal. It is always easier to start with "Mrs. Smith" and have the teacher say, "Oh, please call me Sarah," than it is to start with "Sarah" and accidentally make the teacher feel disrespected.
Na…….these days I thought the students address the teachers as “bitch”.

My GF has been an elementary school phys Ed teacher for 35 years. I call her Ms. (last name) all the time.

We always did this out of respect for the system and the teacher.
But the real reason was to communicate (by example) to the children that teachers are to be respected.
