not the worst but one that doesn't get mentioned often is Paul Newman in The Long Hot Summer. Joanne Woodward's accent in that movie was pretty awful too....

Lots of good answers most I agree with, so I'll add Shelley Duvall in The Shining - she was so annoying that I actually WANTED Jack to kill her.
