You have to be a huge raging a-hole for that to happen
Not really. Doctors fire patients all the time for non compliance. They can be very nice but non compliance discharges happen all the time.

This is a good opportunity to reflect on how you're talking to people.

got to be a pretty yuuge a-hole to get fired by an optometrist
Not really. Seems really hard to get an appointment at ones around me. They don't appear to need any business.

Wait wait wait. We're all just going to stroll right past the fact that this came to them on a FAX? What year is this?

you don't want a doctor who's not open to you taking an active role in your healthcare
I had a doctor once who wanted to put me on a certain medication for a micro blip in my blood labs. I declined saying that in context of the full panel it didn't seem to make much sense and it was something that would need to see how it trends over multiple sample. He drew himself up and puffed out his chest and said "where did YOU go to medical school?" I laughed at him and said YOU"RE FIRED.

The only way I've ever seen a patient get fired ia being a total Karen or Chad. Sounds like you're an ultra Chad

You sound like a bitch to complain about some simple paperwork you have to fill out once a year. The optometrist was doing you a favor by asking you to find a place that's more suitable to your needs. Why the frick should the optometrist fork out a bunch of money to purchase software just to please your whiny arse?

I'm sensing a pattern here. OP may be justified. Any way, go somewhere else.

Big assumption here but if it went down like you said. .. again… big assumption… why would you want to go back to someone that doesn’t take criticism not even directed at their skill?

Optometrist have so many questions, and they only want one of 2 answers; it’s this better, or worse, or ( #1 or #2)OP confused the shite out of her with other suggestions

The message was clear: I had the gall to make such a suggestion, inferring that her management style was beyond reproach.
EVERYTHING for healthcare providers has become more difficult in the Covid era. They simply don't have the capacity to deal with anything added on to their plate.

If you get fired by your optometrist do you get unemployment benefits?

The next day, I received the following faxed statement from the optometrist:
Who still sends/receives faxes?
