There's like 10,000,000 alpha numerical codes to put in the computer blanks that determines reimbursement. 10010A is different from 10100A and from 10001A.
Go ahead and try typing out those about 30 times day each and see how many times you make an error. Insurer (BCBS) only knows what the code input says though.
Based on the "co-pay" and "other insurer" statement I'd bet THAT is the coding error. Some mistake on co-pay/co-insurance code.

Happened to me for services rendered in Sept. I think they’re low on cash and trying to through a tough spot or something.
Go online and fill out a form saying you don’t have other coverage. They will pay.

Determination is part of something called Coordination of Benefits (COB).
An example of determination from a carrier.
In this situation BCBS has determined they're secondary to some other carrier. That EOB from the provider is for what they got back (and not in this case) from BCBS. That would mean they would have received payment from another carrier. If BCBS ends up being secondary, the provider only gets the two payments up to the allowable amount. They aren't "double dipping".
You still may not be on the hook for anything more. Ask the provider if they received payment from another carrier. If not, go back to BCBS and contest and tell them you didn't have other coverage (i.e. covered under a family plan if you're under 26, or under your wife's plan if you're married).

That’s a legit possibility. The other legit possibility, that you need to really hope isn’t the case, is that someone stole your identity and opened an alternate insurance account in your name. Just hope it’s an evil insurance company and not an evil rando from god knows where that you have to deal with.

Had a 4 day hospital stay this time last year. On the 3rd day the hospital business manager informed that BCBS had denied me coverage for the stay etc. Had to call from hospital bed to BCBS to clear up the COB. I was real happy with BCBS.

Not sure what rock you've been living under or why you may have just been lucky enough to avoid it, but I've been filling those forms out for BCBS for many years now.
