Knew a cute girl in the 90's who had the worst breath ever. Turned out to be tonsil stones, I had never heard of them before. She had her tonsils removed and it cured it.

Make a burner email account and tell her anonymously. Use the email to give some hints that it’s one of her female friends trying to help her out. Say something like sorry for doing this anonymously but we go way back and I don’t want it to hurt our friendship.

I used to get those fairly often. Not much you can do about them. It really sucks having such a problem largely out of your control.
Yeah, I'd try to poke them out with an orange stick, would try to cough them out, press on the tonsils to squeeze them out... was too old to really consider a tonsillectomy.
Have pity and stand back next time.

If I dont know the person well/cant be honest to them; if I have gums/mints on me, I'll pull some out and start chewing on them. Then I'll start to put them away and go, "Oh shite, I forgot to offer you some. Do you want any?" -- regardless of what they say -- I'll reply, "No, I feel bad here" and just force one onto them.
