Is it because there aren't enough handicapped spaces for other employees? If not...what does it matter? If she has the tag, and there are enough spaces there shouldn't be an issue.

Nothing he's said amounts to harrassment. What does the ADA say about this? Given rampant abuse of handicapped tags, are the owners of the parking spaces not allowed to inquire regarding legitimate use? If she's using her mother's tag, she is breaking the law.

A competent HR office will not approach anything that could be construed as a violation of HIPAA. Reasonable accommodation relies on voluntary disclosure.

Which in theory should be right.
If it is a properly registered tag in her name and is unexpired, this should be a dead issue.
If the tag is a fraud, expired, or is for someone else, not so much.

I had knee surgery in March and my doctor filled out some forms for me to take to the tax office where you get your license plates to pick up a handicap placard. My wife drove me over and the placard expires in 5 years, which I thought was crazy. I’m still having some issues with the knee so I use it about 30% of the time but I’m sure some people ask why I need it as I don’t limp that bad.

If you think it is fake or using someone else's placard, you can call the non emergency police line and they will come out and ticket if it is a fake or unlawful use of the placard.
I wouldn't question her, that's just asking to get your company sued. She only has to tell her employee about her disability if it is effecting her work or is making it unsafe to do her job. She can volunteer it otherwise.

You could be nice and make conversation with her without accusing her of anything. Just say hey I noticed you have a handicap tag, my cousin just got his license and he has a handicap and was wondering what is the process to get one of those tags? Then just casually ask her what her handicap is
