Unless you are on the short list for a job, I'd make it fit one page.

As an employer I'm not reading 2 pages. I want the specifics of skill sets and experiences that relate to my small business. (Medical office)

When I need to hire another doctor or np or pa or medtech I, as a professional, don't want 2 pages of bullshite. Tell me your license and employment history with references. When we talk I can find out where you went to school and personal interests etc.

Good info..
Most likely I'll combine 2 previous and similar positions into on single paragraph, yet still list the dates of each position at the header of the paragraph.
I can probably shorten my military experience to one sentence. Just being in the service alone is all that really matters anyway.

I want it to be to the point. The fact someone is licensed tells me they "should" be able to do a specific skill.
With the work history place the name of a reference for that historical job.

Always put military experience front and center. The skill done in the military and years served (and if went to a military academy or school etc)

No. One page only. Only exception is for a senior leader and then it is called a CV.

The CV is all I know now, and those get rather long with publications, presentations, and such.

Most managers are going to scan over your resume, make sure you have a few of the experiences you actually list and then go on to do other work. Off the bat, seeing a two page resume could cost you the job.

Yes. When I'm hiring a working professional with a decade or more experience and they hand me something that I'd expect from a new college graduate, I lose interest quickly.
It's 2018. If it's worth me knowing put it in your resume. I'm not calling you for an interview to see if you're qualified. If I'm interviewing you, I already know you're qualified.

I think it depends on your level of experience and industry. My advice is always to make it extremely clear what you do so that someone skimming your resume can easily qualify that you fit the basic requirements of the role in 30 seconds (more like 10) or less. Most people are really only looking at your last couple of jobs and degree/certifications to be honest.
The type of resume format that I hate the most is the summary format. When someone writes a bunch of bullet points but makes it really vague what experience was gained at which job I immediately delete. I don't mind two pages but I'm not going to try and sift through a bunch of b.s. to figure out what you did and where. 9/10 it's an attempt to mask job hopping or a lack of career progression.
