Not sure if he qualifies as the villain in Pulp Fiction, but Vincent Vega leaving his gun on Butch’s counter to take a shite was a really stupid decision.
Buffalo Bill not attacking Clarice sooner when he had a huge advantage over her in the dark basement.
Fitzgerald had Hugh Glass dead to rights and couldn’t finish him.
Bodhi could have ended Johnny Utah’s FBI career way sooner.
Many more come to mind, but there are a few.

Voldemort made several horrible decisions:
1) Entrusting his diary Horcrux to Lucius Malfoy telling him it was a weapon that could open the Chamber of Secrets.
2) Burying the locket Horcrux in his grandfather's house.
3) Hiding the Diadem in the Room of Requirement that anyone can really find and a Ravenclaw student would have certainly immediately recognized it.
4) Trusting Severus Snape.
5) Toying with Harry Potter in the graveyard and not just killing him.
6) Bringing Nagini with him to the Battle of Hogwarts and also weaponizing Nagini.
7) Forcing Harry Potter to sacrifice himself for his loved ones in a near identical way that his mother died.

The big Nazi in Raiders not keeping situational awareness regarding the plane's propeller. Always seemed avoidable. Also, the main villain in the same movie sealing Indy and Marion in the tomb rather than shooting them and throwing them down.

Honestly, they were all fricked anyway. When you really think about it, the Ark was going to kill them all anyway.

From the cobra’s perspective, the cobra not seeing the plexiglass sheet that prevented the strike on Indy when he first hit the ground.
