This place, first issue
LDH said, "Food consists in whole or in part of a filthy, putrid, or decomposed substance.
Wtf
Also
Food was put together or held under unsanitary conditions.
Ready-to-eat potentially hazardous food prepared and kept on premises for over 24 hours that is not date-marked.
Food stored for cold holding and service was not kept at 41°F or below.
Raw animal foods that require different cooking temperatures were not separated.
The interior of the ice machine was found to be unclean.
